Contrast and quiet zone
Dark code on a light background, with a clear margin, is the most reliable.
Size for distance
Phone scanning needs a code larger than about 2 cm; bigger for table or wall use.
Test the proof
Scan the proof on screen and from print before you approve.
Use a stable link
Printed codes cannot be changed. Use a link you control, or a redirect service you trust.

Matte finish on white stock avoids glare that can defeat phone cameras. Avoid gloss for codes and avoid very small sizes.
